Divine Manor Care, Inc. offers ‘5 Levels of Care.’ We are here to meet your needs; whether it’s from mild assistance to maximum assistance, including a special dementia program and hospice supportive care. Our care team will meet with the potential residents and their family members (or POA) to develop the Resident Service Plan to address their care needs. The care team will monitor the effectiveness of the Resident Service Plan and make changes to accommodate the Resident accordingly.
